As a part of the annual registration renewal process, you are asked to complete the Pharmacy Facility Workforce Survey as a self inspection or audit of your organization’s personnel and skill mix.
Please attach the completed document to your completed annual renewal registration form when submitting to the Council for processing. This information will be used to update our records and your facility inspection data.
Additionally registration forms (PDF) can be accessed by navigating to the Resources menu.
INCOMPLETE APPLICATION FORMS WILL NOT BE ACCEPTED AND RETURNED TO THE APPLICANT!!
